"I'll Give Him A Gollum In His Throat"

How did Smeagol ,or if you prefer Gollum, begin making that awful gollum sound in his throat??

In the Two Towers, Sam said "If I ever meet that creature I'll give him a gollum in his throat". He refers to hurting Smeagol. So could it be when Smeagol murderd Deagol he was some how hurt in a deffensive act that it caused his throat to utter "gollum" every time he gulped. Seems Possible I guess. Or could it have been a brawl with a goblin in the Misty Mountains. In The Hobbit it refers to Smeagol to have eaten goblins when he had the chance. What puzzles me ,though, is it also claims that every one knew him as Gollum. Who is everyone though??? The goblins didn't know of his exsistance, or did they??? Could it be the Great Goblin knew of the awful Hobbit under the mountain? Every one could also be the Hobbits of Smeagol's village. That makes a lot more sense to me. I can't see Mr. Tolkien refering to goblins as Every ones. It more like Every thing. Yet I guess "gollum" can have a certain charm to everybody.
